Our Memorial Shadowbox was created to allow the baby or child to occupy a special memorial in their honor to be displayed in their home.

Details needed for Memorial Keepsake

- Baby Full Name
- Date of Birth
- Time of Birth (specify if it's am or pm)

- Date Child or Infant passed away

- Sex of Baby (specify if the baby male or female)

- An Ultrasound picture or infant or child picture
- Baby Weight (specify in lbs and oz)... ie 7 lbs and 12 oz 
- Baby Length ( specify in inches)

I will also need both parents full names, the name of hospital the baby was born at, and the city and state. Also I know that it maybe hard to say but I will also need the details of the memorial was due to miscarriage, still born, during child birth, or later on in life.

All of this information can be put in the check out Note Section during checkout.
